So with lots and lots of fiddling I seemingly, 'randomly' solved this issue for myself. FPS still isn't perfect but it isn't unplayable as before.

The CPU program he uses had no effect. However, I used the console command to raise max FPS, restarted, and now the issue doesn't seem to be there anymore? Hit ~, type "sys_maxfps 200"
Of course make sure Vsync isn't on. I also turned off my Nvidia adaptive Vsync.
