Joinning or Eliminating Forsworn ?

What happens when you join them, and did you kill Madonach or joined him

I only left him alive my first playthrough because I didn't know that you could kill him.

Only for that one quest. Other than that, no you can't. All Forsworn are hostile.

There are three ways to deal with the quest.

Spoiler
1. Kill them.
2. Escape with them.
3. Escape with them then kill them.
