So.. the Hill of Suicides, Xedilian, being responsible for the madness of every denizen of the Shivering Isles.. seems pretty sadistic to me.
I think SI accurately represents his duality of manic and demented.
Like most crazy people they appear calm and non-hostile but when they finnaly strike, their madness shows itself.

He has two sides.The happy,cheeful jester...And the powerful homicidal insane maniac.
His insanity is infectious, a contamination to all others. A great threat, if you ask me.

Sheog just like messing with people. If looking harmless allows him to mess with more people's heads, then that's what he's going to do.

One has to wonder if the Mad God is really as addled as he (And others) would let on. Hes rather composed in the Accords of madness, and even his seemingly random tasks during the greymarch have reason to them.
I actually felt that he got more reasonable as the quests went on. Perhaps because he was slowly turning into Jyggalag.
Or because it'd be impossible to write a questline with a point with a completely unreasonable task-giver.
That's not madness. That's just having a bit of fun at Haskill's expense.
