If his helper would take over his shop after his demise I would let fate take its course if a dragon attack or vampire attack killed him. But given how much I use his shop, and I don't think anyone will take it over if he dies, I'd save him.

One time, when i was a werewolf, I accidentally ate Belethor. I actually missed him. Its really inconvient to not have a merchant in Whiterun.
