I can think of a few things, but I will make suggestions based on whats possible within the limits of the game's engine/mechanics.

A viral outbreak.

An earthquake.

An unidentified object (comet of some kind) causing a fiery explosion.

A snow storm that covers more than half the map and draugr rise en masse (game of thrones style baby!).

A RAGE mutation that causes all NPC's to Frenzy.
