They should've implemented those in DC suburban areas. I don't know why they didn't.
These were built by the tons in the 50's and 60's. Most were not much more than a hole in the ground with a door on it. I would imagine that most were filled in during the 70's. So, your best bet is to buy a house that was built in the early to mid 50's, as that has the best chance of having one of these in place.
As for cities, it is harder to see them now, as most of the signs are gone, but there are many bomb shelters in cities, but those are mostly sub-basemants of buildings.

I would assume some form of bomb shelters that civilians either made themselves, or bought are around. Probably very expensive, and was probably cheaper to try and get into one of the vaults.
In real life? There are tons, though probably not like that in the link.
Some people buy shipping crates and combine them to make a place to live in, in case of a nuclear war irl. Iff I beleive correctly, some people have bought old ICBM bases to use as houses.

Real life they are definitely a real thing. It doesn't seem like the people of Fallout were as bright though.
In the 70's, the US started selling off old ICBM Missile bases. Used to be a cheap way to get a lot of land out in the middle of nowhere. Although it's kind of scary just how close to major cities some of these missile bases were, like the one outside of Castle Rock, CO. I have been to that property for a party. He had not finished it to live in it yet, but it was nearly done. Quite large and roomy. He just had to fill in the missile silos with water (don't want to drop a couple hundred feet into one of those) before he could get his permit to live in it. He was trying to figure out how to get the few hundred thousands of gallons of water cheap and how to filter/maintain it so it would not go stagnant or smelly.

There is an example of a civilian shelter in Fallout 2 located in Golgotha.
It would be great if they had more of them. People have been building backyard fallout shelters since the 1950s.
