Somehow I have no where to map my right-hand to. I'm not sure how it happened... is there a "reset controls to default" option somewhere? Everything works, but LMB no longer uses my right hand. The mouse works fine, all the other buttons work, and in-menu the LMB works fine, just seems to have been unmapped to right-hand in game.

Ideas?

Okay, tried a new character, same problem. There is no "right hand" option in the control menu.

The answer is on your screen shot, T for Defaults, but that might only show you the defaults.

You could also try deleting the INI or Prefs files (Prefs probably has your key bindings in it) and let the game recreate them. That should give you the default mappings back. Delete the ones in My Documents/My GAmes/Skyrim, not the install directory.

Note, if you have any ini tweaks, they will have to be reentered if you do this.

C:\Program Files (x86)\Steam\steamapps\common\Skyrim\

Delete ControlMap_Custom.txt.

Navigate to:

C:\Program Files (x86)\Steam\steamapps\common\Skyrim\Data\Interface \Controls\PC\

Delete controlmap.txt.

Now just run Skyrim and setup your keys the way you want.
