It seems that you have to hold down the left anolog stick in order to sneak, unlike in Oblivion and Skyrim where you only have to press it. Either I'm blind, or there's no option to change this.

Is there any strategy you can give me to sneak efficiently without giving my thumb cramps?

On the Xbox you can double click the anolog stick to lock in sneak mode.
