You get asked by the leader of Riverwood or the Blacksmith of Riverwood to tell their Jarl (the Jarl of Whiterun) of the attack in the hopes of being sent assistance...which the Jarl does in the form of three guards. In other words, it makes perfect sense.
Walking around Windhelm in legion armor or Solitude in stormcloak armor is definitely...odd.
Speaking with Ulfric... as Altmer dressed in full elven armour. Not a single [censored] was given that day.
Walking around Markarth in Forsworn armor, with a character aligned to the forsworn faction. Even stood in and listened to the jarl and his steward complaining about how the forsworn can not be beat in their homes, that they are everywhere. The conversation ends with the steward saying "They live here to my lord." meaning in the city. Which is true.
And then the Jarl sells my forsworn character the house in Markarth and says her imprisonment was a big misunderstanding. Is that your way of saying "We're scared of you and we're SOO SORRY!"
Being able to walk through Solitude in stormcloak gear or Windhelm in imperial gear.
Being attacked by thugs while the city guard watches... only to have them walk up when the fighting is over and say "I'm going to find out who did this!"
And this is why the steward of Whiterun tells the Jarl sending soldiers to Riverwood might cause the Jarl of Falkreath to think they are planning to go to war with them.
Riverwood is close by and you warn the Jarl of Whiterun to prepare for an attack on it.
Yes, Helgen is in Falkreath's possession but the reason you inform the Jarl of Whiterun is because the news brings more guards to the defenseless Riverwood, which is in Whiterun's hold. Makes sense IMO.
Joining the stormcloaks as an Altmer while wearing thalmor robes...I've done that twice.
Making Ulfric while wearing Ulfric's clothes then signing up for the legion.
Killing 20 people and going to jail for like 3 days for it yet, In another quest (won't say which) a guard captain gets a little sick from some mead and the guy who served him gets life in jail.

Speaking of that, I wonder why nothing more is said about that. Falkreath doesn't react at all to Whiterun sending guards to Riverwood. Sure, Siddgeir isn't the most competent Jarl (read: least competent Jarl), but Proventus shouldn't be worried about nothing.
The underwear is actually quite consistant. Everyone's wearing it and it's all the same kind of underwear.
That's not what skyrim on YouTube leads me to believe
The Stealth system is Skyrim ultimate inconsistency.

With 90-100 in sneak...
Wear Elven or Glass armor... In other word some of the most shiny and reflective materials. (almost no leather or fur, and a shiny fresh look of metal) and...
- Sneak 20m in front of a dragon and all he will ever do is looking at you with a straight face. (The eye icon will be barely opened)
- Touch the nose of a freshly transformed hostile werewolf right in front of his/her face.
- Open/close doors and take things magically right in front of NPCs. Why not stealing their clothes/armors while your at it?
Alertness :
Shoot an arrow in the face of anything humanoid NPC and if he survive and don't find you, while having the arrow in his heart or face like the ultimate percing, will say "Must be my imagination".
Air resistance is always better:
Get into sneak position and start walking. You will walk slowly.
Get into sneak position and start rolling (if you got enought stealth). You will move faster, but at adrenaline cost.
Get into sneak position and start... jumping! You'll move as fast as with the rolling, but at no adrenaline cost!
Get everywhere faster while staying out of the view of anything!
Might as well mention the "Red fish syndrom" :
Kill an NPC and any NPC around will move in and check what happened. After a while, they will forget anything about this dead NPC... but... kill any NPCs in area that don't "restart" and the NPC will stay there infinitly. (Even if it's a vampire... Always good to see a vampire corpse in underwear in the middle of a town for 9 months... days and nights... That until a new random assault appear in the same area.)
Even more, the inhabitants will always say "What happened here?" while moving close to the said corpse... every time they pass next to it even if they pass next to it 30 times in a day.
To be honest, if I had to say something about inconsistency anything related to the player's character is inconsistent. Just think at this : Many wars and fight around that last for the last 50-200 years. It takes one hero to clean the crap around and fix everything like if he/she was a greater god itself or herself.
Well, if we're going to take all the mods into the equation inconsistencies are limitless.
