NPCs cause extreme slowdowns

I went back to Oblivion recently jsut to find out that for some weird reason my game that runs at 60fps most of the time slows down to about 30fps or even less every time an NPC is near (it doesn't even need to be showing on the screen). This happens even in Vanilla but since i'm using mods that already bring down my FPS it ends up getting even worse.
As an example, i'm using the quest mod Blackwood Company by Lingwei and i had a quest where i entered a small cave where Bandits and Marauders were fighting. There was not even 10 people on each side and they were in different sections of the cave but still my FPS went down to single digits. Ok... This was after installing a ton of mods but still my FPS is perfectly fine, even getting to 50+ on very busy areas with no NPCs like the areas Unique Landscapes messes with. Anybody has any idea about what could be causing this? Looking on Task Manager i don't see any CPU core going any close to 100% and since running graphical mods doesn't seen to change the FPS at all for me i would guess it's not the GPU too. Oblivion also does not even get to 2gb of RAM usage and disk usage is barely 50%.
My computer specs are:
AMD Phenom X6 II 1075T 3ghz
AMD Radeon 6770 with 1gb VRAM
10gb DDR3 RAM
This is my mods list, it's quite extensive, but like i said the problem still persists even on vanilla Oblivion, it's just aggravated by all the mods i have:
