Well, before I or most posters give an answer, are you on console or PC?
I do a "challenge" game every now and then. My rules for these games are:
No Magic of any kind, including spells, scrolls and Shouts
No Enchanting, and no pre-enchanted weapons, armor, clothes or jewelry (these get sold)
No Alchemy, and no pre-made potions (these get sold)
No Smithing
No helms
No shields
No followers
No fast travel
I almost always play a Light Armor, 2-Hand Warrior type of character when I do this. These characters are only allowed to level three skills and they're only allowed to put perks into those three skills.
Try not to use potions, as mentioned above. That makes things very challenging if not playing a character utilizing restoration, especially if you're playing on Expert or above.
A good challenge that without mods and/or Dawnguard, which you may have but is still challenging, is to play as a vampire. No health/magicka/stamina regen during the day + being severely lowered can be risky, especially if you're a level 4 and don't have Dawnguard.
Yeah, it's pretty rough starting out. On master/legendary, levels 1 through probably 6 or 7 get insta-killed by two-handed enemies, which makes killcams unbearably frustrating. But, if you learn to play it right - also know as running around while your follower whacks away at their health - you will eventually become hardened enough to survive two hits. That's how you know you've attained true power.
Oh sure, at a certain point and once you've amassed enough health either through enchantments or leveling, one can simply just left-click/button mash 'attack' their way through the game. The hardest the game will get is Legendary from level 1-20, after that, it's pretty basic.
There are several things I do to make the game more challenging. I don't fast travel, and at the start of the game, I choose four skills. Those are the only skills I'm allowed to put perks into for the rest of the game (not including werewolf/vampire perk trees). I play on PC, so I can use mods, and I always use Frostfall, Realistic Needs & Diseases, and several mods that make dragons tougher. I also use SkyRe, although I'm going to use Requiem on my next character. I play on adept difficulty.
So true! On Master, I can survive a 2-handed power attack when I'm blocking (with 10% health left) and finally feeling somewhat resilient
