If the quest "http://elderscrolls.wikia.com/wiki/Glory_of_the_Dead" has been finished and the Dragonborn has become the new Harbinger, the guards in Whiterun will start to address the Dragonborn as Harbinger and show respect. However, if another faction quest is started, the guards will no longer recognize the Dragonborn's status and only address him or her as the latest Companion recruit.

Is the Dark Brotherhood & Thieves Guild the only factions that will affect this, or will anything that's considered a possible "faction" disrupt it (i.e. Imperial Legion, DLC content, etc.)?

Guard dialogue is broken, if you enter the College during the MQ then the guards will sometimes say "you're that one from the college, heard about you". I sometimes get the respectful greeting of being a Harbinger from Solitude guards even after joining another guild or the Stormcloaks.

This is one of those gripes with the game that I believe the unofficial patch fixes.
