NPC won't activate pull chain

I have a guard NPC that "should" activate a pull chain to open a portcullis when player approaches. The set up is as follows. Player enters a triggerbox and sets a stage 220. NPC's AI package is "Activate" and conditioned to stage 220, it is at the top of the NPCs package stack the only other package is a sandbox which runs 24hrs and is below the Activate package. All looks good when player approaches, NPC moves from his sandbox markers, either idle arms folded or lean against wall. He looks at the pull chain but does nothing. Magically the portcullis opens without any interaction from NPC. His package must be running or the portcullis wouldn't open. I guessed it was an animation issue but I've seen NPCs, specifically Karliah in Twilight Sepulcha, activate/use a pull chain when escorting the player and Brynwolf inside.

Any ideas anybody? Thanks.

what do you have set for "times to activate" or how that went in your activation procedure?
