game doesn't install past 40%

I got a physical copy and I tried to install the game on the in-game install screen but every time it gets to 40%, it stops working and says that i have to clean the disk. I cleaned the disk like five times by now and there is no way it could be that. Please help, I don't know how to fix this.

I'm on the same boat. I've got my disc changed at the store but the same thing happens with the new one. Tried removing all the data from previous instalations, unplugging the ethernet cable, do a full ps4 reset (turn it off completely and back on again),... Nothing has worked.

Do you think getting the digital version will work because it's a problem with the disk or do you think it's a problem with the game/console?

Do you own both versions? I don't know what the problem is. I hope is a game problem, otherwise it is a lens problem. That means losing my ps4 for a month...

Nope, I don't own both versions because I don't want to wast $65 TWICE on a game that doesn't work. I'm currently asking a friend of mine who works at a game store if he knows what the problem is but so far I can't find any solutions.

Finally managed to make it work. All i did was connecting to the internet again and retry. The day one patch was already installed if that helps.

I also contacted Bethesda support. If they tell me something different i'll let you know.

I've got the exact same problem as you guys. Physical copy of the disk. Install keeps crashing around 50%, give or take a bit. Says it can't read disk. I've tried reinstalling about 6 or 7 times now. Still with no success.

I have the same issue, occurs at 41%.
I've tried just about everything that is under my control, it still keeps telling me that the disc is damaged or dirty. I'm going to try to return it tomorrow. Unfortunately I ordered it so now I have to wait to play the game until I can get a replacement.

Also having the same problem with my physical copy. I've gotten as far as 60% before it throws out the "Disk cannot be read. Make sure the disk is not damaged or dirty." message, but it has also happened at as early as 3%. Most of time it's around the half-way mark. I've also had an error saying that the application data is corrupt, and to delete the data and reinstall. I've tried rebuilding the database, deleting and reinstalling, waiting for the patch, ignoring the patch, and just restarting the console several times with no luck. I also tried installing a few others games to make sure it wasn't my PS4 - all the other games installed without any problems.

Sometimes, when I 'Close Application and Remove Disk' like the error pop up says, then put the disk back in and retry, the instillation will resume from where it left off, but other times it will restart from zero. It just crashes again within a minute, regardless. I have to wonder, though, if it's still actually installing. Even after the error message pops up I can hear the SPECIAL videos playing in the background, taunting me.

I have the same issue. I have the physical copy of the disc. When I try to install the game it errors-out at 11% some times at 4%. I have the latest system software update, and I downloaded the day one patch. I have deleted the Fallout 4 saved data, and restarted the PS4. Same results every time. It keeps telling me data is corrupt, eject disc, delete file and try again. No love.

I may just return the game and wait for the GOYT edition after they've worked out all the bugs. :( What a bummer.

Well, tried a brand new disc and had the exact same problems. Gave up and went digital.

Took the game back to gamestop and got a replacement. Then I decided to return the unopened replacement and got a $59.99 credit. Will wait for Fallout 4 GOTY edition and hope they fixed everything by then. My refund money is going towards a game that works.

I'm having a similar issue but not during the install. The game seems to install fine but once I start playing I get the disc read error at the same part of the game every time. I've got a thread on here about it and I've submitted a help ticket to Bethesda but only received an automated response. I don't know if I can get a new disc because the one I have came with the Pip-Boy Edition, and I do not want to send the whole thing back to Amazon because I'm sure it's irreplaceable. I've considered going digital too, but I'm not certain if that will fix the issue and plus I'll have to spend another $60 on top of the $120 I've already spent for the Pip-Boy Edition.

Did anyone find a fix ? i have got to 68% but usually its around 40 when it crashes.

This is killing me, contacted bethesda and got some autoresponse and Tweeted them but no reply.

Interesting, i found a similiar problem from last year. Wolfenstein the new order for xbox one had the exact same problem, another bethesda game :(
