I only hear audio when I press the Xbox home button or pause

When I try to create a character, I can't hear anything except the radio playing music in the background. Then, when I hit pause or the home button, I can hear things happen and the game progresses slightly. I continued to do this until I created a character and started walking around my house. But whenever I would talk to Codsworth or try to activate any of the items around the house (for example nuka cola in the fridge) nothing would happen until I pressed pause or the home button on the controller. I am currently reinstalling the game to try and remedy this.

Its an audio settings issue. I had the same thing happen to me. Installed from disc, launched game, no voice audio. I dug into my audio settings on the Xbox. Apparently after the forced UI "upgrade" from Microsoft they saw fit to turn on a bunch of Optical and Surround Sound audio settings that my TV doesn't even support. Once I reset those, everything is fine.

disclaimer i don't know if this will fix it but just something to try if you haven't already, try a hard reset on the xbox by holding down the power button on the console for about 5 seconds until it turns off this has been solving a lot of issues across the board. hope it gets fixed for you.
