I just tried it with my level 16 character with 20 hours playtime, and it worked great, so I doubt it's related to play time as suggested.
So my previous post here explained how I set it up, but here is a more detailed explanation including screenshots.

1. Start the game on your PC, enable the Pipboy app in your settings, save and close the game. (once you have it working you obviously don't need to do this)
2. Start your Pipboy app. You should see this screen - http://i.imgur.com/TFARWaa.png
3. Now start your game and load into your save file completely. At this point Fallout 4 has launched with the app setting enabled, and Windows should pop up with a firewall warning requesting permission. You may need to alt-tab out of the game to click OK on that firewall window.
4. Next click on Connection Setup in the app, which goes to this screen - http://i.imgur.com/mcRnlny.png

6. a. If your PC has been found on your local network you will see this screen (IP address may differ) - http://i.imgur.com/hPipfyU.png
6. b. If your PC has not been found you will see this screen - http://i.imgur.com/TtuungM.png. This means either your setting in your game is not on, or your firewall is blocking connection.
7. Click your PC IP address in the app and if all goes well you will see a success message, and your pipboy will display - http://i.imgur.com/2ebYKl5.png
Hopefully this is helpful. Like I said above game time doesn't seem to matter, so it's something to do with your local network or phone/PC firewall settings.
I loaded a 9h,30mins save and it connected, I loaded a 10h save it didn't, I loaded a 9h,30min save it connected, I loaded different 10hr, save and it didn't connect. I didn't touch my network in that time (my house is hardwired on a 200mb connection, with 2 AP points.) same device (ios), same network, all tested within 10 mins of each other.
It isn't a network related issue in the slightest (in this instance.) others may appricate the guide, but as I'm a tier 1, 2 & 3 IT engineer, I can only wait for an update on the app / game.
Thank you for the testing however, hopefully helps narrow down the cause.

I'm studying a degree in Computing & IT, so I have an understanding of networking as well.
If it was a straight up game issue with length of play, my 20 hour save wouldn't have worked. So there must be something else involved here. I'm not saying it's not the game, but clearly between you and I something isn't aligned quite right.
Do you have any theories? Does your PC display on the pip-boy app at all, just not connect?
Well, I'd agree if it was nonsense. But, it's not. I tried it without those port settings in the firewall as well. That's the first place I started. If you read my original post, I don't think the PC is the issue. The PC responds to the phone app's broadcast. It is the app that doesn't receive the unicast message from the PC telling it that it is available for connecting (just FYI, I've been in IT for a long time and this isn't my first rodeo).
I am using AV. However, I have tried everything with it disabled as well. On a side-note Norton AV hasn't been an issue in the past with things more complicated than this.
Btw, for the android Pipboy app that I have, I don't get an option to "click" on Connection Setup. The app boots. Shows a Pipboy loading screen, then immediately goes into a "Connection Setup" screen where it shows 5 buttons. PC, PS4, XBox One, Offline Mode, Demo Mode. Offline Mode is grayed out.
When I press PC, it searches and then says no devices found.
The really odd part... So, as I'm tapping PC on the Pipboy app each time, I see an entry in Wireshark's capture on the PC. PC receives the broadcast and signals the phone. Same as before. There IS SIGNAL!
Odd thing is though, I installed tPacketCapture on my Sensation and tPCAP on my phone was also capturing packets while the Pipboy app was running. However, it shows no such packets being sent out. Seems tPCAP doesn't capture all ports, just common ones? Just guessing... Anyone have a better app suggestion for packet capturing on a Droid?
I also have a Samsung Galaxy Tab 10.1, but the Pipboy app won't run on the ROM I have installed. Pipboy comes up and then crashes before it gets to the OS loading screen.
So, starting a new game also didn't work. This svcks!
same exact problem.... Solved I turned my router the computer is plugged into, to a wireless acces point and now my app works... weird huh
maybe the network after all perhapps just in configuration
I havent a clue but it wasnt working for me before I fiddled with my netgear router... an tadah. Basically eliminated my netgear from the loop. Wonder if he has it windowed fullscreen or borderless. alt-tabing mine closed connection to my app
Hi everyone,
I just found the fix to this problem! I honestly did not try any of that port stuff but after reading everyone's complaints I noticed there there is a communications issue with the PC and the hand-held devices. Diving into the setting on my phone i found no issues, however going through my wireless setting properties on my PC, I found that the "Allow other network users to connect through this computer's Internet connection" was disabled. Enabling this fixes the communication problem.
This can be found in the "Network and Sharing Center" - "Change Adapter Settings" - Right click your wireless adapter - "Properties" - "Sharing".

Hey, everyone. In my case, it was because my AV solution included a firewall. That didn't get modified to allow port 27000 and port 28000 through. I manually added them and viola! The app started working. I'd recommend that anyone having an issue test first either by disabling all firewalls on their PCs; or modifying the firewall rules on the Windows/3rd party firewall, then try to connect.

No go. Considering I started a new game and it didn't work, I thought that your save file may not work either. It's something to do with the app. It has to be. Unfortunately, I can't tell if the unicast signal is even making it back to the phone. Seems the packet sniffer I use on my phone doesn't catch broadcasts, maybe not anything above port 1024...?
So, I installed Shark for Droid and saw that the broadcast is making it out of the phone (already proven by Wireshark on the PC). However, there's no incoming entry for the destination or source port (even though the PC sends it). I've tried everything from disabling firewalls everywhere to creating a PAT on the router. I even upgraded my phone's ROM to the latest stable version. Ever since I did that, I now get a prompt in the Pipboy app as to whether I want Full Screen or Hardware Pipboy. That's new. So... I know something isn't jiving with the app...
