Plug unplug headphone causing lock-up

Anyone else having this issue?

I plug in my headset while in game, and it causes the game to lock up.

Or.

I UNPLUG the headset while in game, and it causes the game to lock up.

Is your system set up to switch the Default Audio when plugging your headphones in? Might be a setting in the control panel of the headphones.

Is it a USB headset or 3.5mm.
