Something just happened that really made me mad, and I'm hoping it was a random one-time thing so I'm curious if this has happened to anyone else.

I was clearing out an apartment building close to DC, starting on the roof. There was an elevator that took me to the roof where a Raider was camped out, sniper style. I took him out and made sure that the roof was clear, before I went in through the hole in the wall (no load zone). As I entered I suddenly took fire from behind, turned around and to my surprise saw a Raider with a dog, who promptly killed me in my unpreparedness. I was like "Dang, where did they come from!" So I loaded, cleared the roof again and walked backwards into the building lo and behold, just as I crossed through the opening a Raider and a dog materialized out on the roof and attacked me. I literally saw them fade into existence before my eyes. I noted that the elevator was still up and they didn't spawn anywhere near it either.

What kind of crap is that?? This is exactly what Beth said their world ISN'T like. NPCs are supposed to be following their routines and living their own lives while the PC roams around and messes things up for them. This kind of thing I can see in a FPS where it's just "moar enemy, wow, kill!" but it does not belong in an open world setting like Fallout =/
