Permanent injuries

I'm not talking about a scripted quest sequence, but something that happens in any combat. You may lose a limb or get disfigured, and you go the rest of the game with a makeshift prosthetic or just go without it, and your dog can help you out. Unless you're able to find/afford a cyborg replacement limb from some doctor of the Institute or other high-tech organization? Think of it as a super cripple feature, the new limb you get has advantages/disadvantages depending on what type you get (a robot hand may make you stronger but less agile).

I'm sure I can fire a shotgun or fat man with one arm.

I wouldn't mind if they did that for a hardcoe mode, but for a regular play-through, it would really annoy the crap out of me. Beth has already stated they want to keep players from resetting as much as possible. Losing a limb would certainly call for a restart.

But seriously NO. This game need no super realistic gimick of a gamplay element that just will end up anoying you everytime the RNG throws out the "You get dismembered" variable. Nor do i look forward to spending 400 plus hours only to become a terminator/cyborg at the end.

Leave this kind of stuf to games who's purpose is to BE though, like Dark Souls and such.

I voted only if done right. I would be very happy to see if a enemy NPC hits my Player Characters (PC's) hand with a sledgehammer that my hand breaks and I have to use one hand to fire the guns like rifles instead of using to hands to fire the rifles, until my Player Characters (PC's) broken hand bone is healed. I would be very happy to see bruises and cuts make it harder to use guns and other stuff as well. Deep cuts should make you bleed a lot.

No, but I wouldn't mind seeing NPCs who have old battle wounds, and whatnot, with some missing limbs or fingers. Maybe an eye or two. Both human and ghoul, and super mutant.

That's not a bad idea. It certainly would be different than what Beth has done in the past, where every single NPC's body is identical to each other (including granny in Riverwood having the body of an 18 year old girl). Differing bodies (some obese, some emaciated, some muscular) would definitely add a much needed extra layer of depth to their games, missing limbs would be great, but not for the pc. IMO, that's going a little too far in the realism category.
