Leaving a building during combat.
Instead of breaking agro from the fight to collect your self...Every single agroed mob that was in the building no matter how remotely away through a maze of crap is now instantly at your feet.
It is like the act of exiting a building creates a space time vortex instantly pulling everything out and into the same 20 cubic foot space.
This is wrong on so many levels.
1: Breaks immersion by doing some Doctor Who level of creative Sci-Fi tech.
2: Is an instant suicide instant.

Hip-firering.
Just plain not working no matter what gun it is...more shots go wild then even hitting in the same zip code unless placing the thing in their face.

VATS aiming, just useless. Worse chances to hit then Hip-Firering. And this is Aiming Assistance >< Where is the Assistance ??? With out perks or weapon modding this should be the most accurate form of shooting.

Go through a building once and have booby traps every where...die and one gets you. Reload and not a single trap any where. This is more immersion breaking

AI tracking you and shooting at you through walls, floors and what not. I can't see them through walls...what gives ???

AI have endless grenades, really they never stop spamming nades, it's like they got a FULL backpack of nothing but nades with no weight limit.

Stealth sneaking combat. There should be at least one entrance of every zone where it is not covered by Spotlights, Turrets, Tincan chains and other instant giveaway crap.

Companions running around every where waking up everything, opening every unopened door, knocking crap over, running into things, getting in your way, teleport following you right off a cliff and incapacitate them self.
