I've been having issues playing my Fallout 4 disc. The game worked perfectly for about 3 weeks and now console won't read the game

i've tried other games and they worked perfectly, the fallout disc is in perfect condition, i've been to the store i brought it from and did a straight up trade for brand new fallout 4 today and the problem still occurs

the messages that keep coming up when trying to play the games are

Unrecognisable disc

Corrupted data

I've also tried uninstalling the game and installing it again

I also did a system rebuild on the console and still no change and also the playstation restart fix

Any ideas on solving this problem please

Just wondering.. are you trying to use old saves or does it happen when trying to start a completely new game? I had this happen with The Last of Us after I put in a 2tb hard drive. The console was just acting really weird with the disc.. not recognizing it and what not even though it'd let me install the game. Once I deleted all the save files associated with the game it worked fine. (Deleted game, then deleted all save files associated with game, then reinstalled game.) No idea why save files would do this but that's what worked for me with that game.

Yeah i'm using old saves because i'm really far into it and has taken me ages to get there, if it's the only solution available i'll have too try this but don't really want to start over again

Well I would recommend backing up your save files somewhere, then do the delete, if it solves the issue then you know it was the save files. If it doesn't solve the issue then at least you know you have your files backed up.
