I got the quest Dunwich Borers in my first saves and also got Kremvh's Tooth and two mini nukes. I got some problems and deleted all my saves and started on a hole new save. When I got the Dunwich Borers quest again the knife was missing, at the alter there was only the two mini nukes. I am sending this in so you may fix this on the next patch. I have read more people with the same problem so you may already heard about this issue.
