Game was glitched, deleted it, is now glitched again

(This was originally a mail to the support but i figured posting it on the forum too would help even more. Would be awesome if anyone has any idea of what is going on.)

Hello,
I pre-ordered my Fallout 4 online because I wanted to be able to play it the same morning as it was released. So I did, and I played, until one day when my game completely lost it. I was doing the "Explore the Combat zone" quest, and when I was done shooting I got into a conversation and then I left. I then turned off my ps4 (after saving, of course.)
But when I got back to playing something was wrong. It was as if I hadn't done the mission at all. I stood outside of the door and entered, and at first nothing happened. I walked far into the room before I was recognized and shot at. However, when I was being attacked I took no damage, or at most very little damage. After this the game was just generally glitched. Sometimes when I was waiting or sleeping, and the hour countdown was ticking, the countdown would suddenly stop for no reason. It could stay on one number for 10-15 seconds before moving on. Furthermore, when I would ask doctors to heal me they would do so, but after they were finished I could ask them to patch me up again, paying the same amount of caps even though I was already healed. I still took almost no damage (compared to what I did before) and several minor, absolutely not mayor but small, annoying glitches that I found gamebreaking were constantly present.
So I deleted the game. I downloaded it, started it up, made my face and fixed my stats, in and out of the vault and voila, everything works perfectly fine.
At least it did. Last time I reached level 25 before the glitches arrived. This time I reached level 11.
Once again it is minor things. Occasionally I stop panting when I run, and just run until I stop. When I had leveled up and was picking a new perk the "perk music" wasn't playing. Every time I equip up my laser musket I have to charge it up, even though I just unequipped it fully loaded. Finally, when I run in first person with a rifle equipped I see my right arm as it is covering a big area in the bottom right of my screen.
I have browsed around and can't find anyone else having this problem. Nevertheless I find it really strange that I got this issue twice.
