I am unable to play the game in full screen mode on proper graphics settings it reverts to lowest quality if I try to play in full screen.

Try playing the game in windowed mode with the fake fullscreen. Some people will have better results using windowed mode and some people have better results in full screen.

Also it might be due to the ultra wide monitor.

Try to go to your Fallout4Prefs.ini file located C:\Users\[Your Username]\Documents\My Games\Fallout4

and check if it's read only !!! (right click on that file - Properties (unckeck Read only)

It is not read only, I would think a popular game such as Fallout 4 would be able to run on ultra wide

Open your Fallout4Prefs.ini file located in: C:\Users\[Your Username]\Documents\My Games\Fallout4

and then see if you have those lines like this below:

Save the changes.

I will give it a try, however it makes things easier, I can do full screen however it auto switches it to awful resolution

You might want to set it to read only after changing the settings in the ini file

I believe you can add those lines in the custom ini as well and just leave the prefs ini as default settings.

As you can see from the above changes on the Fallout4Prefs.ini file that's on a Windowed Borderless screen.

So try those, you may get it to work on Windowed Borderless (Fullscreen)
