Sorry as I'm sure you get plenty of these threads. But I've been away from Morrowind for awhile and am looking to start up again ... as such I'm looking for a good NPC Head Replacer (most I know of include the ability to use the various different heads for your own character) other than Better Heads.

What would be your recommendations? I'd prefer to avoid anything to out there, anime'ish and the like.

The only disadvantage of a pluginless replacer is that it does not offer the head variety of a mod like Better Heads.

And keep an eye om http://www.gamesas.com/index.php?/topic/1093073-wipz-westlys-head-replacers-still-working-on-a-better-title/, which is looking really good. The Breton and High Elf faces have already been released in that post somewhere, and the Nord and Orcs too. IIRC I was not yet using the orc replacer in the screenshots above but I could be wrong...

I'm using better heads along with Westly's Head Replacers.

I use Better Heads as my base replacer, but then I spent a few days adding some of Emma's and Westly's heads into the mix, especially on NPCs that I see a lot.
It takes a long time, but it's a great way to customize the NPCs in your game.
