I've had supply lines cut off, then ported to the town where the provisioner came from and they respawned. I didn't have to assign a new one.

I usually equip my provisioners with some type gear suited for the wasteland. A suit of helmeted spike armor along with a upgraded double barrell shotgun and a few stimpacks. He's always made it back no problem.
