It is a "bug" (old driver related) where you don't clear the dust properly from the pip-boy when you pick it up the first time in vault 111. For some reason the dust stays there. As seen here; http://imgur.com/gallery/Wj2TyID/new
It is easily corrected by updating your Nvidia driver, and if that fails, only reverting to an earlier save will do.

Load an earlier save if you have one.
Alternatively, you can try use the console commands to unequip the pip-boy and then equip it again.
Fallout 4's pip-boy 3000 Mark IV have the ID: 00021b3b
Unequip:
player.unequipitem 00021b3b
Equip:
player.equipitem 00021b3b
(I haven't tried unequipping the pip-boy before, so don't know if it just falls to the ground. If that is the case, just pick it up and it should re-equip, or use the above command)
You can try remove it entirely, and then respawn a new pip-boy, using the commands;
player.removeitem
player.additem
Note that added items will spawn in your inventory, but since it is the pip-boy, you will have to use the command player.equipitem for it to show.
