A mini-DLC where you visit the manufacturing plant for Jangles the Moon Monkey and find a gang of ghoul children who murder advlts and eat them.
They have a super mutant slave who they whip around the factory to do their bidding!! When he sleeps they set feral ghouls of him to torture him.
You can either side with them to meet their master, the original Jangles who has incorporated an AI into his head making him a super genius; or you can side with the mutant butlet/slave and murder everyone; or you can teach the children (who happen to be 280sh years old) to be children and not murder people.
Maybe thats a bit much...

The teleporter opens up a lot of potential wacky ideas influenced by Star Trek and The Twilight Zone. You teleport in a radiation storm and the next thing you see is Nick and Codsworth with goatees with the opposite likes and dislikes. Imagine teleporting to an alternate universe and meeting the inspiration of Grognak, or arriving in one where the Chinese won the war. What if you arrived in Skyrim, took on step towards Whiterun only to find out that you on a movie set.
Out of all these, I kind of want to see Codsworth with a goatee.
Synth versions of the Unstoppable's and quests to go along with them that allow you to get them as companions. Also with Dr Brainfreeze as the villian.
Or take it a step further for the alien lovers and have Killer Clowns From Outer Space
(yes that's a real movie and you should watch it if you're afraid of clowns)
I would be happy with a new, exotic area. of any kind! Love these post-apoc worlds, but couldn't mankind (and creature kind) still find a lush jungle or pine-filled forest to populate? add in any local beasts and baddies!
I would love to see some type of alien oriented DLC, but heavily influenced by 1950's B-Movie styles/tropes/visuals.
Not sure if this would work in Fallout, but it sprang to mind.
Underwater location in Lake Ontario.
Secret underwater facility that was created pre-war, as a refuge to survive the war. Could have started as a Vault, but then expanded into an underwater city, of sorts. Run by descendants of the original Vault dwellers, they have become accustomed to the high pressure living environment (engineered to be that way by Vault-Tec? - part of the Vault experimental purpose?) and can no-longer survive topside without pressure suits.
Power armor mods: underwater propulsion.
Weapon adds: Harpoon gun. Shoots through an enemy, if it doesn't kill them, it tethers them to a spot with a 10 ft length of cable. Slow firing, but so much fun!
Wacky: The whole thing is very James Bond. The current overseer has read some James Bond novels and fancies himself a Super Villain. Cats and bad puns galore. He believes that you are James Bond come to undo him.

A group of people fixed up a printing press and decide to publish new issues of Grognak.
Some fans of the old Grognak don't like the first new issue too much and decide to attack them.
You get to decide who to side with.The nice artists, or the nasty raider scum oldschool Grognak fans...
Abby of the road DLC which will introduce knight templars type of organization and a lovecraftian type of a quest where you find an ancient artifact thought to belong to a pagan deity. You can either join with Abbey, or the Cultist/Organization protecing the artifact.
This idea wins. I want a Lost World-esque adventure - complete with giant dinosaurs.
