I have a misunderstanding that bothers me.
How can anyone survive 200 years?
The bombs fell in 2077, ok?
In Fallout New Vegas DLC Sierra Madre Dean Domino is alive after 200 years.
More precisely we are in the year 2281 and he is still alive.
How can anyone survive the passage of time 204 years ???

Ghouls do not age. A person that becomes a ghoul can theoretically live forever, assuming no one kills him/her. Violence is basically the only thing that can kill a ghoul.
That's why Dean Domino is alive. Radiation ghoulified him and with a little skill and luck he has survived over two centuries.
