When he breaks down click on him and for 2000 caps he gets revived.

If you're talking about when he falls to the ground in combat, he'll just get up on his own after a little bit. You can't restore him to 'new' (or pre-war)condition or anything though without mods.

Edit - Disregard this. I saw it in the sidebar and didn't realize it was for Fallout Shelter.

You can't fix him until he dies completely. Then you can rebuild him.
