A decent idea for a Fallout game.

I would like to see one in the setting when the bombs fell, say, you play 2 days and on the third day is when the bombs fell.

You might want to watch Threads (search youtube).

Thing is, there is so much chaos and confusion, that there would be no established entities...no quests, no factions, etc.

That would be pretty cool, but not only when the bomb falls. Say you start off three days before the bomb and you do minor quest's for people to prepare for the Bomb, or just random quests like delivering items. Then BAM! the bomb falls and you wake up and look around the wasteland, and start the game

So? how about a fallout game that is just about surviving. It would take place over about 30 years and you would see how the united states changed after the bombs fell.

A. Die from radiation from the bombs.
B. Get stuck in Vaults for many years because you need to wait for the radiation to clear up a bit.
C. Become a Ghoul.

They have. Vault 29, for example, was opened as early as 2090, and Vault 8 in 2091.

Not everyone outside of the vaults died, this seems to be a common misconception. Places like the Hub, Junktown and Megaton were founded by people who survived the Great War, as well as many tribals and raiders.

There are hundreds of better ideas for the next game imo.

It'd be boring to see a bunch of bombs fall and then doing nothing.
If they make it like other FO games, it wouldn't make sense.

The Majority of the people you meet in any given fallout game did not come from a vault.

It's a different kind of game...a total survival game, and probably a "me against the world" type thing. Nothing wrong with that, but it's not the kind o setting that supports, as I said previously, factions, quests, politics, and Heroic Myth. It's difficult to see how a single individual could really accomplish anything that would impact the world.

I'd say start 20 years after the fall, at which time some social structure would be present.
