I've run into a new(?), interesting bug.
No matter how accurate I am, no shot I fire will connect outside of V.A.T.S.
I know some will say that I'm just not aiming right, but if I'm standing literally in front of the target, and my weapon is touching them, I'm pretty sure when I fire, my bullet should connect.
I've tried anti-personal, armour-penetrating, and even regular rounds, but nothing will hit unless I fire-up V.A.T.S.
Anyone else experiencing this issue?

I have not had the problems you are having but I have had accuracy problems with using iron sights at long ranges (mostly with rifles). If I'm aiming with the sights and the round doesn't hit the target, what is the point of using the sights? It should hit the target if you are aiming at it.
