So you might have noticed the latest update decides to completely IGNORE your vsync setting?

Modify Fallout_default.ini in your INSTALL DIRECTORY (.../Fallout New Vegas/) to iPresentInterval = 0

The launcher will then write your Fallout.ini with that value every time and all will be good.
