So when you fast-travel, your horse generally comes with you (not inside a city, though, I know). However, I have been fast-traveling here and there outside of cities, and my horse no longer comes with me. Should I assume it got killed somewhere?

Not necessarily. Check at the stables where you got it. Sometimes they wander off to thier former "home"

mirocu's suggestion is good. And if you still can't find it and you're on PC, there's a mod called "Dude, where's my horse?" out there that'll add a quest that has a map marker for the location of your last ridden horse so that you can find it again.

Thanks, I'll get to looking. I did already buy another horse, so not sure if that mod would work then. I'll go check Anvil where I bought the horse in the first place.

This happens to me all the time. I stable my horse at Chestnut Handy Stables in the Imperial City and when I don′t ride it for a long time it walks away, thinking; "Fine. Be that way. See you in Bruma :stare: "
