I'd go for a shotgun, preferably modified to get the most range and accuracy, even if that means fire rate of nothing.
For a backup, some SMG with fairly low rate of fire, medium-high damage, large clip, and fair accuracy.
Shotgun for split-second encounters, ripping off a large chunk of health in one go, most likely in an ambush.
SMG for prolonged encounters, like if I see someone coming at me, I use the prolonged window of damage an automatic weapons gives.
I wouldn't use any weapons suited for long range, as a Medic's job is to get in, heal, retreat. He's very intimate with the action, and needs firepower focused on ~8-15 feet.

Also, It's a fully customizable game. Some people just want to rock with revolvers. They're supposed to be completely balanced, so it all boils down to preference and situation.
