Weird Graphical Glitching

So I've just started to have these strange glitches, I can usually play normally for like 30 mins to an hour when I first start up my PC, then all of a sudden either this glitch occurs or it crashes, and every time after that when I try to play the game, the glitch occurs after like 30 seconds of gameplay about 80-90% of the time. I never used to have these issues with the game before, it's just started acting up. It's a little difficult to describe the glitches but take a look at these screencaps I took:

Sometimes I can still see most of the objects in the game it's just kind of like a strange negative image, or swapped palette, while others as you see are completely blurry and the colors bleed horribly. I can generally play fine with the glitch for a few minutes, but then it will either freeze my PC or just crash, so I usually exit as soon as I get them. The glitch changes rapidly as well, as an example the screencaps I took were all taken within like 30 seconds of each other.

I haven't added any new mods lately (all current mods have worked fine in the past). I even attempted reinstalling my graphics drivers with the latest drivers, disabling background processes (using Advanced SystemCare 4), and turning off UltraMon (my dual monitor program). I have added RAM recently though I have no issues with other games or anything having to do with RAM.

All Official DLCs (GotY DVD Edition)

Unofficial Patch + DLC Patches

BlackWolf Backpack
CALIBR
DCInteriors
NoLockTopic
ProjectBeauty
Underground Hideout
WeaponModKit

All mods have been installed using Fallout Mod Manager, and I've played them fine in the past with never an issue.


PC Specs:

Intel Pentium Dual CPU 2.20GHZ
Windows XP Home Edition SP3
3.25GB RAM (Actually is a 4GB set, but BIOS can only recognize up to 3.25)
768 MB GEForce 9600 GSO

Open up your case and check you video card fans. It's possible it may be overheating. I get some very odd things happening to mine when that happens, quite often followed by a lockup. You might also want to try lowering some of your settings. That card of your isn't all that powerful, you shouldn't be taxing it too hard.
