I have turned footstep volume all the way down, and even with the loud interface sound fix in MCP Im still getting insanely loud footsteps every so often. Why is this?

Two things to try...
Don't turn the sound completely off. Leave it one tick to the right.
Switch between Hardware and Software sound in your main menu. (try the opposite of which ever you're currently using)
