Quest Kick-off Oddities

Hey gang!
Have an oddity with one of my sub-quests, Rescue Rhona.
It is not supposed to start until a bandit at my advanced bandit camp WEST of Whiterun is killed.
That leveled actor has an on-death vanilla script that kicks of the quest and directs the player to read a ransom note.
The quest is not Start Game enabled.

Now here's the oddity:
I am starting new games, with LAL - start at the Windhelm docks, to check out my Dunmeth Pass Border Fort Guard AI packages.
I never even come near Whiterun or my advanced bandit camp.

Out of the blue, after playing the game and observing the Border Fort, my Rescue Rhona quest kicks off!
The one time I visited the advanced camp to see what happened, the bandit was still there!

The leveled bandit actor should never have even spawned, and I did not use a vanilla actor - I created a new actor "DKRAdvancedCampBanditWith Note," so a vanilla actor didn't kickthe quest off.

Any ideas how an actor that has not spawned, is still alive can kick off a quest on a death that hasn't happened?
