While I personally agree and like the idea of helping out the underdogs, it would aslo make the game unbalanced. You cant really have organizations like the Powder Gangs going up agains well armed trained NCR and hoping to win. They wouldn't have the resources even with the couriers help. They are probably there as filler to give you a few side quests or whatever.
Well now, it's true that no single small faction could win - because, as you say, they're underequipped and have less manpower and resources than the big guys. But an alliance of sorts of the small factions like mumatil suggested could work - the BoS could be the heavy troopers/infantry, the Great Khans the light infantry, the Followers of the Apocalypse the medics, etc... And of course, you forget that the PC is a factor here too! We can give'em better equipment AND we can spearhead any attacks, thus keeping loses to a minimum even in the face of a numerically and technologically superior enemy, just like in many of the cases in FO3 (like the slave attack, as Ozzyschach suggested - normally, the formers slaves will lose, but with you helping them, it's another matter).
