Eh.. not too crazy about eating and drinking in RPG's. Mostly i feel this is a nuisance. If you want to make it optional, that is fine. For a "hardcoe" mode, I would expect something like more difficult monsters (higher health, more damage) rather than eating/drinking.
It can be a nuisance, or it can be an interesting challenge to go along with the combat. If you have ever played Betrayal at Antara, you generally had to prepare before going into the wilderness with rations, repair supplies, and for some things, ammo. Sure, running out of something important at a critical juncture stank, but it encouraged you to not do that. Managing your inventory was part of the game.
That said, it needs to be well done - such that it feels like an integral part of the experience, not just an extra hassle. References to food and drink, as well as starvation and thirst, should be woven tightly into the dialog.
I will further admit that the effect is somewhat lost in a single player game. When you have to deal with a party, this becomes a more interesting dynamic. That said, I can see real role playing value in being forced to periodically sleep, eat, and drink. If anything, being forced to setup camp or stop at a tavern can be used as a guise to introduce quests.
