"Heavily modified" by a team with little PC experience in a country with nigh non-existent PC playerbase. Tango modifying the engine to work better on PC is exceedingly unlikely.
Besides, as I understand it, megatextures are pretty much impossible to multi-GPU because the limiting factor is texture streaming rate which is already being hit with a single GPU. To feed two GPUs would mean to halve the already insufficient bandwidth, which obviously would lead to an unacceptable degradation of quality.
4GB VRAM ? That's hilarious.
So maybe if I have GTX 780 3GB I shouldn't try to run this game at ultra settings ?
Graphics in this game isn't worth for this requirements.This requirements is just a cover for terrible optimalization in the game,because only a small group of people will pass this requirements.
If you doesn't pass this requirements,you wouldn't get any help form Besheda support,because your computer is to low to run this game.That's a result for your any problem with the game.Solved very fast.

Sounds like an over the top estimate to me, so I'm not worried if it can run on the older gen consoles, it sure as hell can run on my PC.
Well, you could wait a few days and see how it runs for other people.
You could take into consideration the previous two games that ran on idTech5... and how neither needed or came anywhere close to 4gb of vram.
You could view this as typical marketing for GPU vendors. A game's requirements state it needs really high end hardware... when in fact it doesn't. Bethesda said these are the specs needed to run the game as it was "intended" by the developers. Yet, they don't state what settings, resolution... anything. At they talking 1080p, 4K... Ultra High settings... what?
They said the same thing about Wolf:TNO... yet, people with i3 and lower CPUs can run the game with zero performance issues.
If you're concerned, give it a day. Wait and see what people say once they have it in their hands. But, I definitely wouldn't write off the game simply because the system requirements say one thing. Specially since Bethesda have done this before with Wolf and DisHonored.
