Does this happen to anyone else?

I occasionally will get extra reload annimaions when in VATS even though there is still ammo in my clip.
Sometimes at the beginning of a fight even though I have just manually reloaded, sometimes after I have fired a few round and re-entered VATS.
The enemy continues to advance or fire, and at times this is the difference between dying or not.

Is this common for the PC (never experienced it on my xbox) or a glitch in my load order? Running FWE, MMM, WMK and a few other smaller mods.
