Earlier in the game I contracted the disease followed the Fallion quest to remove it. Later down the road I fought the Vampire lord and contracted the disease again and found that I can not go back to Fallion for removal. How can I remove vampirism?

Only other way I know is to become a werewolf, then cure yourself of lycanthropy if you want to take that path.

thank you friend but that too didn't work..i just completed that quest line threw a witches head on the fire killed my wolf spirit and still a vampire

actually I got it! for whatever reson I had all symptons of a vampire except I did not have Vampire Lord. Had my companion change me into a vampire (Serana) then I was able to go to Fallion a second time for the quest to remove.
