Just a crazy thought for an expansion but what if the expansion included extra areas for the "Bull" (ala Fallout New Vegas), The Brother Hood of Steel (Fallout 3, DC), and ........why not an army of super mutants from Canada. Expand the map a little more in every area and have all three factions try and take over the common wealth and your settlements while you fight them off.
