Well, wright some books like The Real Barenziah (in the original Daggerfall version, that is), and leave the rest to players imagination. Morrowind was such a good game because it fueled your imagination, rather than telling you: "Now you have six with that lustfull argonian maiden (wich, by the way, is a reptile, and thus has a cloaca, wich is, frankly, downturning, but whatever) *insert cheesy six-scene here in wich nothing really is shown and that bears no resemblance to real life six (or bears, for that matter)."
And regarding six as displayed in The Witcher: This it not so far off from reality, i know quite a few guys who do this kind of trophy hunting, it is just that most of the people don't get the chance to [censored] everything they see, thus thinking it is impossible, while it really is not. 1968, anyone?
Edit: To the guy directly above me: Yeah, i think so too, Geralt is a fully developed character, and thus the actions of the NPCs can be finetuned to fit him perfectly, that is not possible in an Open World RPG where everyone is playing a (more or less) different character.
