Skyrim graphic issues.

I've been having a few problems on Skyrim as of late. It all started when I was double casting soul trap. I noticed after a few casts that my hands looked super smooth and detail less. I also noticed the same crappiness on the moss on rocks and trees, the over all look of trees and grass, some prices of armor look bad as well, wood looks pixelated, and the most worst graphic problem that makes me want to not even play skyrim! When I look at my female nord with no armor of, her skin appears scaly and pixelated, while her head remains smooth. I've tried everything from backing up my saves to a flash drive, and deleting everything else. I've also cleared system and game cache. I'm at the end of my rope. Idk what to do anymore. I'm playing from the disk on a 4GB Xbox 360. Please someone help me resolve this issue.

You likely need more free disk space, given that you have only 4GB total on your console's hard disk drive. Even though you are running the game from the disc, Skyrim makes extensive use of the cache on your HDD and when you are low on free space, these graphical tend to start occuring. Try buying an external HDD or other compatible storage device and you should see some improvement in graphical quality. It is best to have 10GB or more free on the storage device that you have Skyrim installed on.
