Strange FPS Issue on Laptop

Basically, just got Fallout New Vegas: Ultimate Edition from Steam, checked before hand if my laptop could run the game with the Can you run it? website and pass everything, same with that I can play fallout 3 and also Skyrim on high settings with 30-40fps.

Laptop specs:
I3-370m 2.4ghz
4GB Ram DDR3
AMD Mobility Radeon HD 5650 (OC to 650mhz/1000mhz for gaming, works well enough and doesn't overheat as much as I would thought it to)
Windows 8 (Only recently installed it, and tried the same thing with my previous windows 7 and had same results)
ATI Catalyst 12.8

Now when I ran the game it suggested ultra settings, but with knowledge of tweaking the ini's that I had to to make fallout 3 and skyrim run as well as they did, I thought i'd give it a shot. So I lowered everything by half, and thought it would run rather well....come to play the game and....I get FPS dips every few seconds effectively cutting my FPS in half for a few seconds until I shake the camera. max fps I got was 25 indoors and < 20 outdoors. So rendering it rather unplayable, even disabling the mods I had got from the nexus didn't help.

After nearly giving up and lowering all the settings manually with the launcher to low, still had the same problems. I decided to let the launcher decide the recommended settings again, once more it detected I should use ultra high, played the game and behold, even with my mods installed....I got 60fps indoors constant, only dipped a few times when loading objects to begin with. And outside depending on how much is being rendered I get 60fps, and if alot is on the screen it dips to ~40fps.

Now why is it whenever I change any setting manually in the ini it reverts back to how it was before? As I wouldn't mind lowering the shadows, and changing a few of the LOD's. Just wondering why, if I change it to anything less than ultra, it just goes skits, whereas on ultra it decides to run like a dream considering its a laptop aswell. Any ideas?

TL;DR, Good fps when on the ultra settings, crap fps when I edit the ini's or any other setting compared to ultra. Just wondering on peoples opinion on this.
