Suggestions for Melee/Survival/Poison build

I just completed a hardcoe game with a Guns/Survival/Repair character and am looking for a new challenge. While playing with the Survival skill I found that there are several interesting poisons you can concoct, but didn't get to play with them as I had a ranged character. If I am to use a "Poison Melee" character, what perks, traits and skills work well with that play style? I'm thinking Sneak would be a useful skill, since it would allow you to approach your enemies undetected, but otherwise I'm not sure what else to take, or what stats to set up. (STR obviously, maybe END, not sure what else would be useful.) Is it better to do melee in VATS or FPS style, or mix it up? How important are crits? Do crits multiply poison damage?

Unfortunately poison is rather broken in this game because every poison you concoct can only be used ONCE. Its recommended to get the Heave- Ho perk because you can then use poisoned thrown weapons as a means by which to silently poison and kill enemies.
Its rather frustrating because I love using throwing knives + tremble poison combo but its hard to get throwing knives. So i have resorted to sporadically going into HH to wipe out White-Legs to get tomahawks.
