These r good examples, but I am positive i can compete w/ great chances of winning DOH Trophy:
- approaching an important quest-giver, left-clicking the mouse and shooting an arrow, instead of pressing the space-bar ... just to hear "STOP RIGHT THERE, CRIMINAL SCUM!" from a nearby imperial guard;
- Collector quest: an Ayeleid Statue, when u first look at it, looks like that support for weylind stones but empty. So I cleared the dungeon (a small one gladly), search for the damned thing, not realising it was right in front of me... and left the ruin, cursing about some sort of bug and abandoning the quest (till much later I saw on the uesp website what the statue looks like indeed);
- forgetting to check on repair hammers BEFORE entering a three-level dungeon full of goblins: I was about to enter the LAST level of a quest to rid goblns (JIm or whatnots), saved and thought "let's keep that bow sharp and ready", only to realise I had no hammers (and the crap weapons goblins had, ha ha, try using them with STR 30 and blunt blade <20);
- As if the above was not enough, I had decided to 'give it a try', u know... Maybe a silver-bow with health 11 can be enough, if I sneak and engage only when necessary... Besides, I was looking very much down on going back two levels, travel to a city, just to buy hammers and redo the whole damned dungeon tour... which was enough to render me 3 more dead monsters, 4 or 5 reloads and the experience of what not to do...
