Well after reading all the ideas, I'm afraid mine just don't compete, but....
1) Giving the aliens anol probes...
2) creating a DLC that gives deathclaws the ability to regenerate to a 10 count....
3) I liked the poo-ing yourself comment as well! Kudos to you! Take it another step farther... make the Brahman leave cow pies that you step and/or slip in (spend half the frickin' game recovering)
4) If you ever get critically radiated, you end up a ghoul.
5) There are no bobbypins. You have to make your own tools that only last for 5 tries... and you pay more than they're worth for the privilege of having them.
6) the DJ only has one record.
7) the Enclave does brain implants and even though you wouldn't have gone in there had you known, you get brain surgery... oh yeah they did that already (Tobar's Brain Surgery for those not knowing). I hate it when they make the character do something stupid. Although I suppose if you didn't do something stupid in the first place, you couldn't call it an adventure (me? disarm a nuclear warhead? ME? ewwwwww let me um.. go somewhere else).
Worst features have already been added for me. First person, real time, one world node, limited dialogue, forced backstory, removal of previous features, voiced protagonist, pointless features that don't belong, minigames.
So I'll go with what's left, quicktime events and railroaded setpieces, although we already got the latter when it comes to the main quest sometimes.
I didn't find it overpopulated outside of settlements either, but each to their own.
I think multiplayer is something that may sound good in theory (as long as it's only co-op), but difficult to implement/balance.
The only place where it's appropriate to comment on intelligence levels is in the game itself. And for the record, I love multiplayer games, as do many others on the forum. I just don't think that it belongs in a game like Fallout 4. Does that make me 'not very smart?' Somehow, I don't think so.

I believe that a smart-move technology might be bad.
The game maintains a record of all of the actions you take. Once the game has accumulated an hour or more's worth of data, it begins to extrapolate from that data what you might choose to do in a given situation. Then, from time to time, the game blocks you from making a choice and instead makes the choice for you based on your established pattern.

Multiplayer, even if optional. You can't be the "sole survivor" if your buddy can bring his "sole survivor" character into your game and explore together. I also play in the Fallout universe to get away from the real world, I haven't enjoyed playing online with others since the Forza series did away with player controlled rooms where you could boot annoying people without taking a vote. I don't even have a Gold account any longer as multiplayer just does not appeal to me in the least.

i use it a lot. actually, i'm in 3rd person almost only when i travel - i mean seriously, how'd you ever aim a gun in fallout 3 in 3pp?
but i think it's pretty perfect as it was anyway, in/out with mousewheel, and close to no pointless cinematic camera wagging (another thing from my personal hate list) - the couple you'd get for criticals etc were just the right dose for me
actually, if it really was "insert coin", i'd find it hilarious
the "enter credit card data" etc it'd really be though - not amused.
Worst part of these game is Microtransactions that allow access to some weapons 50% stronger/faster than standard once.
I used to play one of these FPS MMOGs. Horrible balance. There were some 10$ weapon packs for a week of access to weapons, 150$ permanent.
That is not a way to play a game. svcks the fun out of everyone else who rely on pure skill.
well it's easy to see why this is so: 3d vs iso.
in a top down isometric view, you'll always know where you aim.
in a 3d view with your own character's butt right in your line of sight - not so.
...and that'd be where 1st person comes into play.
